It is a combination of questions obtained from many surveys across the web.
I merged them together then cut it down to the 60 or so questions that are there now to try and accurately reflect the commonality of questions across the surveys.

oh, i am sorry. i like most of it. some of the questions were kind of funny that is all. and some of the way things were asked was a little bit off. it was rather interesting.
